error message with custom questionnaire

hello.
i'm playing with the custom questionnaire feature using html files and am running into some trouble. the questionnaire i'm making has ten free-response items. the questionnaire shows up just fine and the data show up in the excel file (although i do notice that the first variable is only recorded as 'ok' rather than what is actually being entered), but at the end of the experiment, after the "notify your experimenter that you are done" message, i get a vague error message that reads: "MediaLab.exe has generated errors and will be closed by Windows. You will need to restart the program. An error log is being created." Any idea where I'm going wrong? Thanks in advance.

You may simply need to reinstall MediaLab on that machine. Is it happening reliably on this machine and is it happening on any other machines?
