help creating a custom item

How can I create a page of my survey with two or more different questions on it for subjects to answer, particularly when they are different types such as fill-in combined with scale responses?

I have the same problem and would really appreciate any help possible:

I need to create a question like this:

1. The anticipated graduation date is:
a. May/Summer 2010
b. December 2010
c. May/Summer 2011
d. December 2011
Other (please specify)_________________

So, the last will be a fill-in-the-blank and the others are scale items and I really dont know how to do it.

However for this purpose, you might simply get by using the function in your questionnaire. e.g., in the case above, you would enter this option in your scale response:

Other (please specify) <specify>
