how to create a question with a spreadsheet or table

Hi, I need each participant to allocate 100 points to 5 items by filling in a number out of 100 for each item. It could be presented as a table or a excel spreadsheet. However, all 5 items need to be in one page. Your adviced is greatly appreciated.
